To the Board of Directors of Yost Lake Country Club:

I do hereby submit a Cabin Rental Request for the cabin owned by me located on the real property owned by the Yost Lake Country Club Inc., Payne County, Oklahoma. In the event of approval, the renter(s) and I agree to abide by the Constitution, rules, and regulations of Yost Lake Country Club, Inc. The renter(s) and I irrevocably waive all claims against said Club or any of its officers, directors, or members for any act or acts in connection with the cabin rental and/or business of the Club, and particularly as to acts in electing, or failure to elect, advancing, suspending, expelling or otherwise disciplining the renter(s), the member, any member of my family or any of my guests.

I agree to pay promptly upon billing fees, assessments, dues, fines, or other charges related to this Cabin Rental Request.

I acknowledge and attest that:

1. The guest(s) will be provided a copy of the Yost Lake Country Club Rules;
2. the guest(s) are not allowed to have their own guests at Yost Lake Country Club; and
3. the guest(s) allowed on Yost Lake Country Club property are only those listed on the form above and below.

Member & Guest(s) Consent Section

Member and Guest(s) indemnify and hold harmless the Yost Lake Country Club, Inc., its officers, board of directors, employees, agents, members, and its successors and assigns, of and from all actions, suits, losses, damages, claims, demands, and expenses, including reasonable attorney’s fees, arising out of the Member and the Guest/Group’s use of the Yost Lake Country Club, Inc.’s facilities.

The Member and the Guest(s) also agrees:

1. That any damage to buildings, equipment, or property owned by the Yost Lake Country Club, Inc., other than that due to ordinary wear and tear, will be charged to the Member. Any damage shall be immediately reported to a member of the Board of Directors.
2. To assume full responsibility for injury, damage, or loss from any cause whatsoever to the person or properties of others.
3. To comply with insurance requirements and safety and ground rules.

This agreement is entered into by the Yost Lake Country Club, Inc. contingent upon the above conditions and observance of insurance requirements, safety, and ground rules. This agreement does not give the Member or Renter(s) the privilege of including any person(s) or guest(s) or group(s) other than indicated above and on the Cabin Rental Request form.
